Total Creatine Kinase Activity and CK-Isoenzyme Pattern in J. B. Encephalitis Patients


Abstract
This study was carried out to evaluate the usefulness of total creatine kinase (CK) and CK-isoenzyme determinations for a presumptive, laboratory diagnosis of J. B. encephalitis in early stage. Difference of CK and CK-isoenzyme activity between child and adult patients was also investigated.
Total CK activity was determined in 104 cases of J. B. encephalitis, 64 children and 49 adults, and CK-isoenzyme study was carried out 54 cases.
The results were as follows ;
1. Sixty-two percent of tested serum samples showed an increased CK activity. The mean value was 155.8 IU, which was three times higher than control one.
2. Abnormal CK-isoenzyme electrophoretogram¢¥ was noted in 52 out of 54 patients with J. B. encephalitis, the prevalence rate ( % ) of CK, was 35.2 %, CKZ 70.4%
and CK3 74.1 %, respectively.
3. Increased total CK activity was related to concomitant elevation of three isoezymes, especially of CK, and CK3.
4. Adult cases of J. B. encephalitis showed more increased CK activity than that of child cases. Increased CK activity in some of adult cases was remained up to 4 weeks after clinical onset. Prevalence rate of abnormal CKZ was high in adult cases of J. B. encephalitis.
5. Increase of total CK level and prevalence rate(%) of abnormal CK, and CK3 were the higher during the 1st week after disease onset, there after they decreased to normal range. However, prevalence rate(%) of abnormal CK, was on the decrease during the 2nd week after clinical onset, unexplainably.
6. Increase of total CK activity and prevalence rate(%) of abnormal CK, were more remarkable in patients with mental deteriorations.
This results suggested that creatine kinase and isoenzyme determination be useful for an early presumptive diagnosis of J.B. encephalitis and there be certain differences of CK and CK-isoenzyme pattern between child and adult cases.
